We offer a level of care and support appropriate to the individual's needs and embrace the elements of Self Directed Support, as detailed in the In Control jigsaw framework. We focus on enhancing life and social skills for independent living and assist our Service Users in making appropriate choices that help them to pursue their goals and realise any opportunities for further education and employment.
Our Supported Living schemes offer high quality accommodation for Service Users within either their own flat or a shared house. Service Users have their own assured tenancy and are supported to make their own choices and determine their own daily lives.
Social Care Solutions has developed a model of supported living that includes a choice of either low levels of support but unintrusive supervision to higher levels of staff support up to full 24-hour periods to enable all support needs to be addressed. Our Service Users are involved in the process of interviewing and selection of staff. We have an effective system for staff induction and supervision that ensures tenants receive a consistently high level of service.
A full assessment will be carried out to ascertain need and the tenants aspirations and wishes. If an agreement can be reached then prior to admission the referred individual is welcome to visit the project for short periods, meals or overnight stays. Admission will generally take place on a three-month trial period during which a re-assessment of a tenants needs will be conducted by staff.
Emphasis is placed on supporting individuals to develop their social and interpersonal skills and on empowering them to take control over their own lives. This is achieved through Person Centred Plans, which are developed by the individual Service User with support from people such as their key worker, advocate and/or care manager. These Plans provide a structure within which to address individual needs, choices and aspirations, emphasising tenant involvement and inclusion.
Using Key Worker Support skills, individuals are encouraged to discuss their concerns providing resolution and not escalation into areas of conflict or difficulty. Sessions are held weekly on a 1:1 basis, in a relaxed manner and are treated in strict confidence. Additional sessions can be held on a needs or request basis.
Service Users are actively encouraged to distinguish between everyday leisure activities, holidays and work in order to expand upon their life experiences. Community links are therefore further developed through access to local colleges and other educational establishments.
Most individuals require additional support in this to improve their skills and/or to elevate it above being a chore. Initial assessment with the person leads to an agreed plan that is fully monitored. Service Users are supported in planning their weekly shopping list to accommodate their planned menu.
Skills developed and supported include menu planning, shopping, gardening and cleaning. Each Service User is supported in contributing to the general cleaning and upkeep as well as other aspects of the running of the Home. This offers a routing and structure to daily life and the opportunity to develop important skills. Service Users are supported in developing an awareness of the needs of others. Budgeting skills are developed and Service Users are supported as required in this important aspect
